New Customer Request Form Implemented (3/30/1999)
Explanation Of New Form To Customers
Dear Valued Customer:
Starting this week, Logical Operators, Inc. will begin trial-use of a new Customer Request Form. While the form may undergo some slight changes before being finalized in mid-April, we intend to begin using the document for all service calls and parts orders immediately.
Please examine the form carefully during this trial run - it consists of three sections on the front: Work Request (service), Parts Request (parts), and Confirmation of Receipt and/or Completion. On the back of the form is a listing of our revised sales policies.
During each visit from our employees, you will be asked to authorize (via signature of the Work Request) any service prior to commencement of work. If parts are requested or needed to complete a service call, you will also be asked to authorize the purchase of that equipment (via signature of the Parts Request) prior to our delivering the parts. Finally, once parts have been delivered or service has been completed, you will be asked to confirm completion of the deal by signing the Confirmation Section of the form. Upon completion, a copy of the form will be supplied to you if requested.
We realize that filling out any form is a time-consuming (and often dreaded) task, so we have designed this form to be completed by our employees. In most cases, you will only be asked to check the information and sign in the appropriate place(s). This new form will allow us to more accurately document your service/parts order and should help prevent any confusion which may arise during or after an order or service call. Additionally, we expect the information from this form to allow us to produce accurate invoices in a more timely manner.
